Does anyone know of any print-on-demand services that fulfil ALL the following:

a) link to an online shop so I can sell the damn thing?
b) handles both US and UK customers without killing one or other with postage charges?
c) allows me to use my own ISBN?
d) will do spiral/wire binding?

Have you looked at Lulu.com? That's who Diane Duane's using for her self published stuff. Peter Morwood's using them for the re-release of Clan Wars as well.

I'd suggest checking closely whether Lulu satisfies requirement b). A book that I wouldn't mind having came available there recently with a cover price of U.S.$21.99, but getting it to my door in Toronto, Canada would cost almost $25. extra.

They don't give me shipping rates for US or Canada, but to Europe there is a flat rate for up to 3 items of:

1 Item £3.50 or €4.99
2 Items £4.50 or €5.99
3 Items £5.25 or €6.99

which is not extortionate. I would be surprised if their US rates were drastically different. Canada and other countries may be another matter.

The part I was dubious about was the binding, but they are able to do plastic spiral ('plasticoil').

Anotehr vote for lulu, which i've not used myself but have heard good reports about.. unclear on point d) but they have so many options I'd not be surprised.
